Is the primary health centre actually open, staffed and stocked?

The problem

A health centre that exists on paper but has no doctor present, or no medicines in stock, provides no healthcare.

Why it matters

Primary care failure pushes people to distant hospitals and to private debt for conditions that should be treated locally.

Proposed solution

A citizen-reported availability record for primary health centres: opening hours actually observed, staff present, and medicines in stock.

How it would be implemented

Structured citizen reports through the issue system, aggregated by facility, with the facility given a right of reply.

Risk

Blaming individual overworked staff for systemic vacancies.

Mitigation

Report at facility and district level, never naming individual staff. Publish sanctioned-versus-filled post data alongside.
